If you use direct links (both full and thumb) on external image servers, i guess nginx or lighttpd will work. ( i will test this very soon )
If i get the time , ill write a howto.

Hope 2.4 gets options use nginx right of the box, option like yes or no

If your apache is starting to get heavy, you can use litespeed (costs money)
I am using it on a shared hosting site, and it works very well.

direct links here on forum there is already a manual how to do and this is the best solution

Then direct links + ngins - nothing more perfect

I got a pure nginx running on http://i003.imghurl.com
Newest stable nginx with php 5.2.13
    Everything is build from source.

Modified server headings to hide software and version, also php version.
    Use http://www.websitepulse.com/help/tools.php to test
    Code:
    HTTP/1.1 302 Moved Temporarily Server: S2 Date: Fri, 09 Apr 2010 22:47:01 GMT Content-Type: text/html Transfer-Encoding: chunked Connection: keep-alive Keep-Alive: timeout=5 location:
All xxx errors goes to blank or custom error_page.html

    Server Status page http://i003.imghurl.com/nstatus
    No ip-deny for now

    Only tested with direct links.
